In this context, stigma can be understood as the negative perception of people living with HIV based solely<br />on their HIV status. Experienced stigma, or discrimination, occurs when people living with HIV experience negative or<br />unfair treatment based on these perceptions. An estimated one in eight people living with HIV are denied health<br />services because of stigma and discrimination, and research shows that over 50% of people report having<br />discriminatory attitudes towards people living with HIV [1]. Understanding and addressing stigma is critical to achieving<br />the zero discrimination element of UNAIDS vision and successive strategies to end the AIDS epidemic by 2030.<br />To improve the understanding of experienced HIV stigma in the community, ECDC — in collaboration with the<br />European AIDS Treatment Group (EATG) and AIDS Action Europe (AAE) — initiated an exploratory survey in 2021<br />to measure HIV-related stigma across Europe and Central Asia. The objective of this survey was to examine the<br />experience of HIV-related stigma in the region and to establish a benchmark measure of perceived and<br />experienced HIV-related stigma among the most affected communities in order to assess progress toward the<br />UNAIDS target of zero discrimination by 2030 [1]." This discussion paper draws on existing policy and<br />good practice to identify key policy themes and types of interventions for improving community-based HIV<br />and mental health care.<br />The paper also references two EATG-sponsored stakeholder events. The first event, which was internal to<br />EATG, demonstrated that there is already significant experience among members about the HIV and mental<br />health continuum of care. The second event included stakeholders external to EATG. The discussion made it<br />very clear that WHO and others have been working on building a solid HIV and mental health continuum of<br />care for years now. Presenters shared examples of lay workers and clinicians collaborating at the community<br />and institutional/hospital levels, adapting what they do to national needs and the shape of their HIV epidemic." How-<br />ever, data on HIV testing, PrEP use, and PrEP knowledge among trans and non-binary people are limited. We analysed data<br />from the ‘Sexuelle Gesundheit in trans und nicht-binären Communitys’ (TASG) study, a participatory study on HIV/STI<br />and sexual health among trans and non-binary people in Germany. The study was designed, promoted, and analysed with<br />active involvement of community members. Participants were invited to complete an anonymous online survey between 1<br />March and 1 July 2022. The outcomes included HIV testing within the last 5 years, PrEP use, and PrEP-specific knowledge.<br />Predictors for HIV testing were identified using a bootstrap stepwise selection procedure. Among 2468 HIV-negative par-<br />ticipants with information on potential HIV risks, 21.5% had potential needs for HIV testing and PrEP. Of these, only 44.3%<br />(208/470, missing: 60) reported testing for HIV within the last 5 years. Older participants, those living in larger cities, and<br />those with higher education levels were more likely to have tested for HIV. Additionally, only 8.3% (38/459, missing: 71)<br />reported ever using PrEP. Among 451 participants with potential PrEP needs (missing: 79), only 57.4% knew at least one of<br />three key PrEP-related facts at the time of the survey. Our findings highlight substantial gaps in HIV testing and prevention<br />among trans and non-binary individuals in Germany with potential needs for these services. Discrimination has a negative impact on health and may affect sexual health and behavior. We explored the relationship between discrimination based on gender identity and the perceived ability to make decisions about their sex life to feel as protected as desired from HIV and sexually transmitted infections (STI) among trans and non‑binary people in Germany. Secondarily, we assessed whether feeling unable of making HIV/STI‑protected sex decisions was associated with behaviors related to increased HIV/STI risk.<br /><strong>Methods</strong> We conducted a cross‑sectional study using data from the Sexual Health and HIV/STI in Trans and Non‑Binary Communities (TASG) survey conducted online between March‑July 2022 among trans and/or non‑binary people aged 18 years and older living in Germany. We described the prevalence of frequent discrimination based on gender identity. We calculated prevalence ratios (PR) with 95% confidence intervals (95% CI) for the associations between frequent experienced discrimination based on gender identity and feeling unable of making HIV/STI‑protected sex decisions, and between feeling unable of making HIV/STI‑protected sex decisions and behaviors related to increased HIV/STI risk.<br /><strong>Results</strong> Among 3077 participants, 22% reported frequent discrimination based on gender identity. Participants experiencing such discrimination reported 1.4 times more often to feel unable to make HIV/STI‑protected sex decisions (PR 1.4, 95% CI 1.1–1.8). This perceived inability was associated with increased prevalence of sex under drug influence (PR 2.9, 95% CI 2.3–3.7) and condomless penetrative sex with multiple partners without PrEP (PR 2.0, 95% CI 1.4–2.9).<br /><strong>Conclusion</strong> Feeling unable to make decisions to feel protected from HIV/STI among trans and non‑binary people was associated with both frequent discrimination and behaviors that increase the HIV/STI risk. Strategies for empowering trans and non‑binary people to assert their sexual decision‑making needs should be explored.</p><p>Martín‑Sánchez et al.
